ABOUT US
Blessed Hands PNCC (Parents Nurturing and Caring for their Child) is a FREE MOBILE Medicaid and BadgerCare Plus benefit program that helps pregnant or moms with newborns get the support and services they need to have a healthy baby. This is a mobile service and Care Coordinators will meet parents in their home, hospital, or any other place the parent(s) choose.

Prenatal Care (PNCC)
Prenatal care coordination services help families identify specific need(s) while they're preparing for a baby. The goal of this program is to access all resources to provide everything baby and parents need before the he/she arrives. Families will be matched with one of our trained coordinators who will be available to support you during your pregnancy journey.

Child care coordination services help families gain access to and coordinate a full array of services, including necessary medical, social, educational, vocational, and other services. A personalized care plan will be developed with one of our trained Care Coordinator’s to help achieve the desire goal you want. Ongoing support during this time will be available until child reaches preschool age! CURRENTLY UNAVAILABLE
